Job Overview
We are seeking a dedicated and compassionate Care Manager to oversee the delivery of high-quality care in our facility. The ideal candidate will have experience in care homes, assisted living, or nursing homes, and possess a strong understanding of care plans and medication administration. This role requires excellent supervisory skills and a commitment to enhancing the quality of life for our residents, particularly those with dementia or other complex needs.
Responsibilities
- Develop, implement, and manage individualised care plans for residents, ensuring their needs are met with dignity and respect.
- Supervise and support care staff in delivering exceptional care services, providing guidance and training as necessary.
- Administer medications as prescribed and monitor residents for any changes in health status.
- Conduct regular assessments of residents' needs and adjust care plans accordingly.
- Maintain accurate records of care provided and ensure compliance with regulatory standards.
- Foster a positive environment that promotes resident engagement and well-being.
- Collaborate with healthcare professionals, families, and other stakeholders to ensure comprehensive care delivery.
Experience
- Proven experience in a care home, assisted living, or nursing home setting is essential.
- Background in nursing or a related field is highly desirable.
- Familiarity with dementia care practices and the ability to manage complex health needs effectively.
- Strong organisational skills with the ability to manage multiple priorities in a fast-paced environment.
- Excellent communication skills to interact effectively with residents, families, and team members.
- A compassionate approach to caregiving with a commitment to improving residents' quality of life.
